<How to Make a Reservation>
(1) Making a reservation online
In order to make a reservation, please access the Hiratsuka City Vaccine Reservation webpage (https://www.covid19-vaccine.mrso.jp/142034/VisitNumbers/visitnoAuth/).
 
(2) Making a reservation by phone (only in Japanese)
Please call the COVID-19 Vaccination Call Center 0120-751-738 (8:30a.m.~5:00p.m.) to make a reservation.

<If you moved to Hiratsuka city after the first and second doses>
・If you received the first and second doses in your former address and wish to take the booster vaccine in Hiratsuka city, please apply for a vaccination ticket. You also need to apply if you wish to take the booster vaccine in Japan but have received the first and second doses in another country.
・You can make an application online or by mail. For details, see the Hiratsuka city’s website (https://www.city.hiratsuka.kanagawa.jp/kenko/vaccine.html).
・The vaccination ticket will arrive in about 2 weeks after making an application. (The ticket will be mailed no earlier than 6 months after your second dose.)

Vaccination Certificate (Vaccine Passport)

<Electronic Vaccination Certificate (Vaccine Passport)>
You can get the electronic vaccine certificate using your My Number Card. You can apply through a special application downloaded on your smartphone to get the certificate in the form of a QR code (electronic certificate).

<Paper Version of the Vaccination Certificate (Vaccine Passport)>
Please bring the necessary documents to the Hiratsuka city hall or send by mail. If your documents are complete, the paper version of the vaccination certificate (vaccine passport) will be mailed to you. It usually takes one to two weeks to issue the paper version of the vaccination certificate (vaccine passport).
